Tur Yoreh Deah by Rabbeinu Yaakov ben HaRosh with the first edition of Beis Yosef by Rabbi Yosef Karo. This sefer was published by Rabbi Yosef Karo himself!

His conclusions were then established as halachah in the Shulchan Aruch.
