A young man once approached the Chazon Ish to request a brachah for a shidduch. The Chazon Ish inscribed the young man’s name and mother’s name and inserted the slip of paper into the sefer Chazon Ish on Kiddushin. (maaseh ish, vol. 6 pg.125)
